CANCELLATION AND “NO-SHOW” POLICY
- We require 48 hours notice for cancellations or requests to change your appointment.
- A valid credit card is required at the time of scheduling your appointment. This card is to hold your appointment only, and will not be charged until the day of your consultation.
- If you arrive more than 15 minutes late to your appointment, we cannot guarantee that you will be seen by the doctor, as this will cause a delay in the next scheduled patients and it may be considered a “no-show”.
- If you fail to give 48 hours notice to reschedule, cancel, or “no-show” to your appointment, your credit card on file will be charged a $50 no-show fee.
Our staff looks forward to meeting you and discussing your goals. We strive to keep our appointments on time, as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on this website so that you can accelerate your check-in process. If you are unable to fill out our patient forms, then it is recommended that you arrive to your appointment between 15-30 minutes early so that you can have ample time to fill out the appropriate forms.
