When it comes to non-surgical treatments for hair loss, it’s important to understand how they work, what their expectations and limitations are, and what possible side effects may occur. Dr. Yaker discusses the importance of six main principles when it comes to any form of non-surgical therapy.

  1. Non-surgical treatments do not bring back completely bald areas. That is the limitation of non-surgical treatments.
  2. While there might be some dormant roots (under the skin) that can be rejuvenated, their main purpose is the following:
    • For some patients the treatments delay the speed in which one loses. Meaning, you will continue to lose but at a much slower pace had you not started treatment. Essentially, you are buying time for researchers to find better, more effective treatments to help limit or prevent further hair loss.
    • For some patients they will notice their hair loss stop. Many patients report that the treatment continues to keep their hair in place despite being on them for years.
    • For some patients, they not only slow down or stop losing hair, but they also notice their thin, wispy hairs re-thicken. By thickening up the thinning areas, you can more easily camouflage the bald areas in between. Therefore, thickening the hair creates an illusion that new hair formed from the bald areas, when in fact the surrounding hairs became thicker. Of course this benefits people who have early forms of hair thinning and hair loss.

Unfortunately, there is no way to predict how someone will respond to each treatment. Therefore, results vary.

  1. Treatment results may take at least 4-6 months to show but can take up to one year.
  2. For certain treatments, compliance with daily use is recommended for you to optimize your outcome. The most common cause of not responding to treatment is the lack of compliance.
  3. Treatments must be used life-long (yes, forever!) in order to maintain results. Hair loss is a chronic condition that can worsen as you age. Therefore, should you stop treatment at any point, you will lose the benefits it provided for you (slowing, stopping, and/or re-thickening) within a few short months and you will continue to lose your hair as you were destined to lose.
  4. Hair shedding with any non-surgical treatment is a normal, physiological process that can occur for up to 8-12 weeks. As the roots under the skin absorb the new treatments, they analyze each hair shaft they’ve created. If the hair shafts are weak and thin, a new hair shaft is created. As the new hair shafts grow in length, it pushes out the old, weaker hair shaft.

The pH level of your skin, including your scalp, is between 4.5 and 5.5, which means it is more acidic. Actually, your skin has a “film” on its exterior called the “acid mantle.” The purpose of the acid mantle is to protect your skin from contaminants, bacteria, and viruses and other environmental toxins. These attackers tend to be alkaline in nature, so the role of this acidic mantle on your skin is protection, which it accomplishes by dissolving the harmful particles trying to penetrate it. This is why maintaining your skin’s naturally acidic pH is important. Using shampoos and conditioners that are too alkaline can destroy the protective acid mantle. Unfortunately, the average pH of the typical name brand shampoo and conditioner is over 6. Because of the damage it does to the acid mantle, cleansing with shampoos and conditioners that have a pH over 5.5 is harmful to your scalp and inhibits proper hair health and thus hair growth and texture.


In addition, each day we build up dead skin cells, oil, sweat and hair products that form a plaque that sits on top of our scalp. This plaque causes our scalp pH levels to rise, leading to certain scalp conditions such as clogged follicles, bacterial infections, dry scalp, oily scalp, dandruff and even hair loss. Also, if you already suffer form certain scalp conditions, you can develop chronic flare-ups. Moisture retention is pH dependent. Maintaining an acidic pH less than 5.5 causes the hair cuticle, the outside layer of your hair shaft, to flatten and seals the moisture into the hair shaft leading to richer, fuller hair. Most shampoos and conditioners have higher pH levels leading to hair shaft swelling which opens up the hair cuticle leading to loss of moisture and hair shaft damage, as well as dry, frizzy hair.

The increase of the male hormone dihydrotestosterone (DHT) onto genetically susceptible hair follicles is what causes male pattern baldness. Finasteride is a medication that blocks testosterone from converting to DHT by up to 70%, which helps control, stop, and even restore hair loss. Here are some facts about this medication:

  • It is the only FDA-approved oral medication for the treatment of male pattern baldness.
  • It is a once-a-day pill available by prescription only.
  • The medication is 90% successful: 48% will have re-growth of hair and 42% will have no further hair loss.
  • It takes 6-12 months to achieve a cosmetic benefit
  • Of patients with male pattern hair loss that do not use Finasteride, 75% will become “much worse” within 5 years.
  • Positive effects from Finasteride can help reduce the extent of hair transplantation (number of grafts required) by stabilizing future hair loss and possibly re-growing hair naturally.
  • Finasteride can be used to improve the result of hair transplant surgery by stabilizing the hair follicles surrounding the transplanted hairs, allowing for a better overall result.
  • Reported negative side effects are limited to fewer than 2% of patients. These side effects may be reversible on dose reduction and tend to diminish over time with continued use.

It is impossible to ensure the potency of formulations made outside of FDA control. Many companies that manufacture Finasteride (generic) are in countries outside the United States, where quality control methods may not be as regulated, leading to less effective outcomes. As a result, Dr. Yaker offers patients a specially compounded version of Finasteride, known as Nutra.Fin. Minoxidil is an FDA-approved topical solution that is used to help stabilize hair loss and promote hair re-thickening in both men and women.   • Twice daily 5% Minoxidil has been shown to regrow hair in around 60% of men and women.

  • This medication can be started 6 weeks to 3 months before hair transplant surgery to possibly prevent or limit hair shedding that might occur after surgery.
  • The most common side effect from Minoxidil is skin irritation and possibly unwanted facial hair that will disappear shortly after discontinuing the medication.


HÄR LAB NO. 1 is a custom-made topical medication for men and women that acts as a hair follicle stimulator. Platelets are a component of blood that contain specific proteins and growth factors. These growth factors once introduced into the scalp, slow down hair loss, re-thicken thinning hair, and can possibly generate new hair growth by stimulating dormant hair follicles. It is also used during hair transplant surgery to assist in healing of the skin and helps stimulate hair transplants to grow while improving hair graft survivability. While PRP therapy has recently received a lot of attention, it has been in use for several decades. PRP is currently used in many areas of medicine, including orthopedic and sports medicine, cosmetic and reconstructive surgery, oral and maxillofacial surgery and cardiovascular medicine, as the growth factors found in platelets have been shown to accelerate wound healing, tissue repair and skin rejuvenation.

When considering the use of PRP to enhance hair growth, it is extremely important that we reach an ideal platelet concentration to achieve optimal PRP concentrations. Studies have shown that a platelet concentration of at least 1.5 million platelets/microliter stimulates follicular angiogenesis (blood vessel formation), which is a major factor in rejuvenating thinning hair. This angiogenesis event also allows for better blood flow, oxygen and nutrients to penetrate the hair follicles. If a person has low platelets and the same amount of blood is drawn for someone with optimal platelet levels, one will not achieve this 1.5 million platelet/microliter level.

Since platelet levels differ from person-to-person and change from day-to-day, it’s important to know one’s platelet level before proceeding. Unfortunately, with current PRP techniques, the act of drawing one’s blood, separating out the PRP, and re-injecting back into the scalp is performed without ever testing platelet levels.  Therefore, if someone were to have low platelet levels their PRP concentrations would be sub-optimal, unless the correct amount of blood volume is drawn. That is why not all PRP systems are the same and because there is no universal protocol, results vary greatly.

This unique software can calculate and validate a patient specific PRP treatment dose based on your platelet levels and blood volume to provide consistently high concentrations of PRP. We offer two treatment options for PRP. One involves three PRP sessions, separated by 6 weeks. The other option is one PRP treatment with ECM. Booster treatments may be required based on results and can range from every 6 to 12 months. It’s important to note that PRP treatments do not bring back bald areas.

Extracellular matrix (ECM) from donated live, human placental tissue and amniotic fluid contain a powerful mixture of growth factors, bioactive proteins, carbohydrates, and hyaluronic acids. This complex network acts as a structural support to surrounding cells. When used with platelet rich plasma, this creates a boost to the hair growth response. As a non-invasive solution, the treatment therapy is a short 60-minute in-office visit. The process begins with real-time testing of platelet levels through a gentle prick of the finger. Next, a small amount of blood is acquired and spun at high speeds in a special machine in order to separate the platelets from the rest of the blood components. While the blood is spinning, a local anesthetic is applied to the scalp and the concentrated platelet rich plasma is reintroduced into areas of the patient’s scalp that show hair thinning. Finally, microneedling is performed on the scalp to create micro-trauma and micro-channels into the skin, which activates the PRP and allows them to penetrate deep into the skin.

Because PRP is made from your own blood, there is no chance of allergy or rejection. In addition, the use of amniotic fluid doesn’t cause rejection or allergic reaction when injected into a patient.


There is no downtime following this procedure, with virtually no limitations or restrictions. Most commonly, patients may develop some redness, minimal swelling and mild tenderness to the treated areas. These symptoms may last up to 48 hours. While results differ for each patient, cosmetic benefits are typically seen around 4 to 6 months after the first treatment, and will continue to improve up to a full year following treatment. As PRP with or without ECM is not a permanent solution for hair loss, repeat treatments are recommended every 9-18 months. It is important to note that PRP with or without ECM does not grow new hair follicles in areas where you are already bald. It is used to help reverse the miniaturization of hair follicles so they will produce healthier, thicker, longer hair fibers.
